We encourage you to select 2 providers from our team to be YOUR care team. We will work with you to have your child seen by your team as much as possible.
Your care team is here to listen to you, answer your questions to help you fully understand your treatment options.
We'll help guide and support you anytime you need to see specialists' or providers outside the office.
We'll ask for your feedback about the office, and how we're doing in taking care of you.
We'll give you tools and support to help you manage your care.
We'll connect you with community resources and care management.
You can get same day appointments with your care team.
We’re open Monday – Friday 8am – 5:00pm and on Saturdays from 8:00am – 12:00pm.
When the office is closed, you can call (704)-982-5437 to talk to the on-call nurse for urgent needs.
There is easy access to your personal health information through our Patient Portal and you can send non-urgent messages to your provider and renew most prescriptions. Questions through the portal during office hours are answered the same day while after hours questions from portal are answered the next business day.

Ask lots of questions! Bring a list with you to your appointment.
Let us know if your child has seen any other healthcare providers so we can help coordinate the best care for you.
Keep scheduled appointments or call to reschedule or cancel as early as possible.
Learn about your health insurance coverage. Ask us if you have questions or need help knowing where to go to obtain insurance coverage. (Medicaid, North Carolina Health Choice, "The MarketPlace")
Give us feedback to improve our care using our suggestion box or complete ourPatient Satisfaction Survey.We look forward to working with you as your primary care provider in your patient-centered medical home.
Advance Care Team
Our Advance Care Team provides:
Immediate access to medical advice via phone and portal and direct access to providers as needed.
Arranges referrals to specialists and facilitates exchange of information between our office and the specialist.
Tracks best care practices to monitor and facilitate this care for all our patients.
